Brief description of risks during operation and maintenance
Plant responsibility is about compliance with all critical control factors during design, installation and operation. In engineering where we have an approval for construction and installation, we need to be sure of a design that fits the safety before use. During construction and installation, we must have the documented proof of conformity with information for operation and maintenance. During operation and maintenance, we must be sure of the integrity of the installation and safe operation.
All incidents and accidents must be reported and analyzed!
The analysis is evaluated on the basis of the following topics:
- Ignorance
- Competence
- Evasion of rules
- Ignorance of rules
- Lack of rules
- Force majeure An
Ishikawa model can again be used for the evaluation.
Corrective measures include the following measures:
- Staff training
- Designation
- Process ownership
- Procedures
- Means
- Changes
- Repair
In most cases, corrective actions are a combination of the above.
All incidents and accidents must be reported and dealt with at the meeting of the Electrical Safety Committee.
